29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Palram 2000, Inc., P3 - On or about December 1, 2016, procedures for the Bryer Extrusion Lines brand D-78224 cross cutting saw were not developed, documented, and utilized for the control of potentially hazardous energy when employees were performing knife blade changing activities. Palram Panel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1088517, citation number 01, item number 02/a and was affirmed as a final order on November 23, 2015, with respect to a workplace located at 9735 Commerce Circle, Kutztown, PA 19530.
