29 CFR 1926.1204(c): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations as described in 29 CFR 1926.1204(c)(1) thru 29 CFR 1926.1204(c)(8) of this section: The employer is failing to protect employees from hazardous atmospheres associated with confined space entry. This was most recently documented on or about December 15, 2016 at the jobsite located at or near 507 Coleman Road in Belton, Missouri. The employer did not develop and implement a confined space entry program. Employees enter active sewer manholes, are exposed to hazards such as but not limited to oxygen deficient atmospheres, flammable gasses and vapors, and toxic air contaminants.
