29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Packaging Area - On or about 12/05/2016 and at times prior, employees were exposed to amputation hazards, in that a chain of the GA 3 line Perpetual BTO Rollup machine (serial # 2064) was not enclosed when employees were operating the machine to wrap carpet rolls.
