29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 12/6/2016 throughout the warehouse, where the employer failed to develop machine specific energy control procedures. Authorized employees performed maintenance and servicing activities on equipment such as but not limited to the conveyor system which is electrical, pneumatic, gravity, and hydraulic. Employees were exposed to amputation, electrical, caught-in, and struck-by hazards.
