29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department, or other group: a) Throughout Facility - On or about February 9, 2017 and times prior to, the employer had not implemented group lockout/tagout procedures when servicing and/or maintenance on equipment was performed by a group of employees, thus exposing employees to caught-in, pinch point, and crushed-by hazards.
