29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the forks lowered fully: a) The Toyota Forklift Model 5FGC25 Serial number 5FGC25-12326 was observed to have the forks raised approximately two feet with the load. This was being used as a work platform for the lathe operator in the south east corner of the production area.
