29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) On or about 11/18/16 in the Bay area; employee used relocatable power tap, which was permanently mounted on the side of the workbench, to plug in two battery chargers for portable tools, a pencil sharpener and cordless phone and was exposed to a fire hazard and burns. b) On or about 11/18/19 in the Bay area; employee used a Belkin brand relocateable power tap, which had a surge protector and plugs for cable on one end and was permanently mounted on the west wall of the Bay area near the bottle return trailer, to plug in a Ryobi bench grinder and was exposed to a fire hazard and burns. NO ABATEMENT CERTIFICATION REQUIRED
