29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Throughout Packing House - On or about November 1, 2016, employees were operating the #5 Toyota powered industrial truck (forklift) in an unsafe condition in that operators/employees were exposed to struck-by hazards due to the parking brake not holding the forklift in place.
