29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that electrical energy had been disconnected: a. Electrical Room - On or about October 19, 2016, employees were exposed to arc flash/blast hazards, in that verification that electrical current was not present and deenergization of circuits was not provided to employees doing voltage testing and replacement of electrical components.
