29 CFR 1910.145(c)(3): Safety instruction signs shall be used where there is a need for general instructions and suggestions relative to safety measures. On or about October 16, 2016, employer failed to provide sufficient safety instruction signage or other written instructions regarding hazards caused by unauthorized access underneath low level conveyors on van lines. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
