29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the PHA team's findings and recommendations: a) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma. The employer did not assure that 2015 Process Hazard Analysis recommendations such as, but not limited to, ensuring ammonia detectors were operable were resolved in a timely manner exposing employees to the hazard of contact with/inhalation of anhydrous ammonia. b) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma. The employer did not assure that 2015 Process Hazard Analysis recommendations such as, but not limited to, conducting emergency mock drills were resolved in a timely manner exposing employees to the hazard of contact with/inhalation of anhydrous ammonia. c) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma.
