29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever necessary by reason of hazards of processes or environment: a) Employees were working in an energized electric panel DPH-40B-2 and 40-B-4 without using personal protective equipment for the face, hands and body. An arc flash occurred causing an employee to be burned on his body. Location: 388 Greenwich St, New York, N.Y., 40th fl mechanical rm. On or about 10/08/2016 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
