29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely. a) Worksite, Warehouse/Production Floor: Employees authorized to operate forklifts had not received training and evaluation in the safe operation of powered industrial trucks in over at least three years; on or about 10/7/16. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
