29 CFR 1910.333(b)(2)(ii)(B): Control circuit devices, such as push buttons, selector switches, and interlocks, were used as the sole means for deenergizing circuits and equipment: (a) On and before September 2, 2016, in the Electrical Vault Room of the Mill Basement, an employee was exposed to serious electrical hazards in that he did not use appropriate methods and procedures to de-energize a 480 volt electric panel while attempting to change out the electrical fuses. As a result, an Arc Flash occurred seriously injuring the employee. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
