29 CFR 1915.1001(f)(2)(i): The employer did not ensure that a "qualified person" conducted an exposure assessment immediately before or at the initiation of operations to ascertain expected exposures in the workplace or during the operation: a) On or about August 27, 2016, in the area damaged during a collision at level A of the floating dry-dock, employees were instructed to conduct Class III asbestos work and the employer failed to ensure that an initial exposure assessment was conducted by a qualified person before employees started to remove a section of duct work covered with thermal system insulation (TSI) known to contain asbestos, exposing employees working to asbestos airborne concentrations that may have exceeded the permissible exposure limit (PEL).
