29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards such as those created by point of operation, ingoing nip points, rotating parts, pinch points, flying chips and sparks: (a) K.B.P. Coil Coaters, Inc. at 3600 East 44th Avenue, Denver, CO 80216: On or before August 9, 2016, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure adequate machine guarding was provided to protect each operator and other employees from hazards such as those created by unprotected cutting wheels as part of point-of-operation of the YODER 36-inch slitter machine, in the slitting department, during the operating cycles. This condition exposed employees to amputation hazards when contacting the slitter's sharp cutting blades. (b) K.B.P. Coil Coaters, Inc. at 3600 East 44th Avenue, Denver, CO 80216: On or before August 9, 2016, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure adequate machine guarding was provided to protect each operator and other employees from hazards such as, but not limited to those created by point of operation, ingoing nip points, rotating parts, pinch points, flying chips and sparks during the operating cycles of the YODER 36-inch slitter, Dahlstrom slitter 48-inch, in the slitting department, not limited to the following instances. On August 9, 2016 a machine operator running the Dahlstrom slitter 48-inch was caught by the unguarded in-running nip points of the rotating coil re-winder resulted in a fatality. 1. Rotating Mandrel between motorized cart and un-winder 2. Un-winder rotating Mandrel 3. Coil stretch batch operation-sharp metal edges. 4. Rotating coil Mandrel re-winder 5. Rotating coil Mandrel re-winder and coil wraps turnstile 6. Retracting hydraulic ram coil push-off 7. Rotating scrap winder motor and spool 8. Operation of up-ender.
