29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 18, 2016, the employer did not ensure that bales of metal handled by forklifts were loaded and moved in a safe and secure manner to prevent the load from becoming removed from the forks. Metal bales were not secured, exposing employees in the area to struck-by and crushing injuries when the load fell from the forklift.
