29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process. This violation was most recently observed on or about August 9, 2016 in the Multi-Purpose Unit at Arkema, Inc. located in Crosby, Texas when the employer did not identify, evaluate and control the hazards of the process that were introduced by the atmospheric discharge from the pressure relief devices on the Sulfur Dioxide tank and KC-1 and KC3. The discharge point was found to be within 2 feet from the operator walkway.
