29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) At the Digital Room, the employer did not ensure that authorized employees used a lockout device to isolate the energy sources at the main disconnect on a Spartanics, Dye Cutter, Model M500-140, serial number 11088, exposing employees to electrical, and struck by hazards. b) At the Digital Room, the employer did not ensure that authorized employees used a lockout device to isolate the energy sources at the main disconnect on a Spartanics, Dye Cutter, Model M500-117, serial number 10373, exposing employees to electrical, and struck by hazards. c) At the Digital Room, the employer did not ensure that authorized employees used a lockout device to isolate the energy sources at the main disconnect a Spartanics, Dye Cutter, Model M500-200, serial number 13011, exposing employees to electrical, and struck by hazards. d) At the Digital Room, the employer did not ensure that authorized employees used a lockout device to isolate the energy sources at the main disconnect a Spartanics, Dye Cutter, Model M500-181, serial number 12330, exposing employees to electrical, and struck by hazards.
