29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Hanna Electrical Contracting, Inc., at 1833 E. Mulberry St., Fort Collins, CO 80524: On and before June 6, 2016, the employer did not ensure that each employee was trained to recognize the hazards of working at elevation and trained in the use of fall protection. Two employees were engaged in electrical work upgrades that required the removal and installation of exterior lighting located near the edge of a flat roof on a commercial building. An employee accessed and worked from the roof without utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 24 feet. (b) Hanna Electrical Contracting, Inc., at 1833 E. Mulberry St., Fort Collins, CO 80524: On and before June 6, 2016, the employer did not ensure that each employee was trained to recognize the hazards of working at elevation and trained in the use of fall protection. Two employees were engaged in electrical work upgrades that required the removal and installation of exterior lighting located near the edge of a flat roof on a commercial building. An employee who accessed and worked from the roof fell through a skylight and landed approximately 24 feet below. This condition resulted in a fatality.
