29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite: The employer did not provide training to employees who worked from a 6-tier high supported scaffold. Employees could not recognize hazards associated with scaffolds, and were exposed to hazards, including but not limited to: falling up to 40 feet from the scaffold without fall protection, and supported scaffold with a height to base width ratio of more than four to one was not restrained from tipping by guying, tying, bracing, or equivalent means.
