29 CFR 1926.959(d)(2): Observer. A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings before the minimum approach distance required by paragraph (d)(1) of this section was reached: At Freymuth Road near Eisenbath Road intersection in Saint Paul, MO 63366 - On or about 05/05/2016, employees were exposed to the hazard of being contacted by electricity, approximately 7,200 volts, where a designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings before the minimum approach distance required by paragraph (d)(1) of this section was reached. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
