29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: a) 43 Park Place Job Site - On or about 5-6-16 - An employee was electrocuted while working from an aerial boom truck that was in close proximity to high voltage power lines that were not de-energized nor protected from contact. In addition the boom truck arm and bucket were not insulated.
