29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: a) On May 3, 2016, employees engaged in roofing activity were exposed to fall hazards of greater than 25 feet. Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 511458, citation 1, item 1, and was affirmed final order on August 14, 2102, with respect to a workplace located at 123 Melrose, Kenilworth, IL 60043. Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 190481, citation 2, item 1, and was affirmed final order on March 15, 2102, with respect to a workplace located at 1100 Fair Oaks Ave., Oak Park, IL 60302. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
