29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes, more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 934 Arch St, Philadelphia, PA - The employer did not protect the employees from falling through the floor holes with some type of fall protection system thus exposing the employees to fall hazards. This condition was observed on or about March 7, 2016.
