29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: The employer is failing to protect employees from struck-by and caught-in hazards while servicing the Imabe Iberica H-60/1800 baler. This most recently occurred on April 14, 2016 and date prior below the sorting conveyor located at 440 J Street Lincoln, NE 68508. Machine specific lockout/tagout procedures have not developed for the baler to explain the proper sequence and specific energy isolation points for multiple energy sources such as the conveyor dropping materials into the top of the baler, and specific points to isolate the 480 volt electrical feed and hydraulics.
