29 CFR 1915.89(c)(2): The employer did not ensure the use of a tags-plus system to prevent energization or startup, or the release of hazardous energy, before any servicing was started. (a) On the LCS 9, employees were performing servicing and maintenance activities utilized a "human lock" or "insight" method of lockout without ensuring an additional safety measure was used to provide equivalent safety available from the use of a lock.
