Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 02/06/98 for two employees attempting to unload three unsecured pallets of shingles from a scissor type elevating stake-bed truck and exposed to a potential crushing hazard created by the lack of guardrails on the truck bed. ANSI A92.7-1981 Section 13.2, item (5) requires that movable guardrails and doors are in place or closed, or both, unless their location requires that they be opened or moved for normal operation of the unit. Among others, one feasible and acceptable method to abate this hazard would be to ensure the guardrails are installed on the truck prior to the truck being driven or elevated to reduce or eliminate the potential for an unsecured load to slide off the truck bed. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Sample Abatement Certification Letter").tion
