29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): 101 North Main Street, Newark, Illinois, west building shop, powered industrial trucks (forklifts)- on or about February 18, 2016, and dates thereafter, employees were operating forklifts without undergoing workplace-specific formal and practical training, and/or evaluation in safe operation of the equipment. Operators and other employees working in the area were exposed to struck-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
