29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer is failing to protect employees from the hazard of unexpected machine energization and/or movements during maintenance activities. This was most recently documented as having occurred on February 15, 2016, at the Stahl Specialty Company facility located at 1301 Stahl Drive in Warrensburg, Missouri, where: (a) employees were performing maintenance on the Laempe LFB50_H Sand Core Machine (#5527) without an authorized person installing a safety pin in its proper (uppermost) position to arrest any potential downward movement of the ram during the maintenance activity, and (b) an employee was performing normal production operations clean-outs of the Core Box on the second Laempe Sand Core Machine, Model LB50_H, Serial No. 110741 which required him to physically enter the core box without lockout / tagout devices being applied by an authorized person to isolate the machine from multiple energy sources including electrical, hydraulic, and pneumatic sources.
