Huron Rd., Green Bay jobsite; a Hyster Lift Truck (Model: S50FT, S/N: F187V24851L) was jacked up with an AC Hydraulic A/S (FJ40) hydraulic forklift jack. No cribbing or blocking was placed prior to work commencing exposing employee(s) to crushed-by hazards.
