29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: Oxidizer Pad: On, or about, September 23, 2015, the company had not developed and implemented adequate rescue procedures for employees who entered confined spaces. Maintenance staff entered the Smith Incinerator to reposition a damper that had misaligned due to a broken weld. The Confined Space Entry Permit Form instructed employees to call 911, however, the volunteer fire department was neither manned nor equipped to respond to confined space emergencies.
