29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): a) On or about 01/05/16, in the Battery Department; two employees greased and lubricated the Back Door Machine and were exposed to caught in or caught between hazards when they performed these activities with only one lock attached to the energy isolation device.
