29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 110 Industrial Dr., Holden, MA Employees were exposed to struck by hazards while operating an Okuma CNC Lathe. a: On 12/24/15, an employee was struck by 25 inches of an unguarded steel bar rotating at 2500 rpm on an Okuma CNC b: On 12/24/15 and at times prior, employees were exposed to caught in and struck by hazards from operating the larger capacity Okuma CNC with the sliding door interlock/locking mechanism being inoperative.
