29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: (a) On or about December 23, 2015, Premier Roofing Company LLC, as the controlling, creating and correcting employer, hoisted Walter Construction, LTD employees in a wooden basket supported by a JCB rough terrain forklift, which was built for the purpose of removing roofing material, exposing the employees to falls of up to 25 feet.
