29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Edge Plastics, Inc. located at 449 Newman St. Mansfield, OH 44902; On or about December 18, 2015, the employer did not assure machine specific energy control procedures were developed before performing maintenance activities on injection molding press #15. Employees were required to work on a hydraulic cylinder inside the doors of the injection molding machine and did not deenergize the press, exposing employees to crush hazards in the event of unexpected start up of the press. b.) Edge Plastics, Inc. located at 449 Newman St. Mansfield, OH 44902; On or about December 18, 2015, the employer did not assure machine specific energy control procedures were developed before performing mold setting activities, on injection molding presses including, but not limited to the 64 Leader Lid Injection Molding machine.
