29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: WORKSITE: The employees, exposed to head injury from falling loads during the course of lifting and carrying bundles of rebar using the bucket of the Kobelco Model SK160LC hydraulic excavator were not wearing head protection. On or about December 8th, 2015, one of employees was trying to hand-grab the above bundles of rebar (loads) by jumping up and down from underneath the elevated loads that were rigged with slings hooked onto teeth of the bucket of the Kobelco Model SK160LC hydraulic excavator. The slings came off the teeth of the bucket, causing the loads to fall, strike and pin the employee to the ground. The employee sustained fatal head injuries from the incident.
