29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: a.) Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On or about November 18, 2015, the employer did not ensure employees were protected while working on the McKinley Rotary Die Cutter #3. Employees were required to perform rewiring work on a motor and did not place a lock on the 480V disconnect before the start of work.
