29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about October 27, 2015, and at times prior thereto; a. Employees in Building A were exposed to chemical hazards while breaking down heat exchangers containing residuals of hazardous chemicals, without being provided with or required to use appropriate personal protective equipment. b. Employees in Building A were exposed to cold burns while dipping heat exchanger parts into liquid nitrogen, without being provided with or required to use appropriate personal protective equipment.
