29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) In the finishing facility, on or about 10/28/15: A Clark forklift emitting hazardous concentrations of carbon monoxide was not taken out of service. Measurements by OSHA showed ambient air levels of carbon monoxide reaching 250 parts per million. Abatement certification must be submitted for this item.
