29 CFR 1926.403(b)(1)(ii): Safety of equipment was not determined on the basis of mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: a) Danville Borough Municipal Authority Storage Site, employees, blasters, were exposed to electric shock hazards in that the 120 VAC light bulbs, mounted on the ends of the blasting nozzles, were not equipped with safety cage or rated enclosure to protect against bulb or housing breakage, on or about October 27, 2015 ABATEMENT CERTIFICATION REQUIRED
