29 CFR 1926.960(c)(1)(i): The employer did not establish a minimum approach distance no less than the distances computed by Table V-2 for AC systems: a) 2998 Glenwood Drive, Camp Hill, Pa - The employer did not establish a minimum approach distance for employees working near an energized 7.2 kV transformer, thereby exposing employees to electrical hazards on or about September 28, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
