29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by ingoing nip points, rotating parts: In the following instances guarding was not adequate or no guards were provided to prevent contact with ingoing nip points: a) No. 12PM Basement, 2nd Press Felt Stretch Roll Operating and Tending Side - The inrunning nip points where the felt passes over the roll was not guarded. b) No. 4PM, Wet End Tending and Drive Side - The nip points between the dryer felt and the upper guide rolls just before the first dryer were not adequately guarded. c) No. 7PM Tending Side - The small rope sheave just before the reel was not adequately guarded. d) No. 9PM Tending Side - The two rope sheaves just before the size press were not adequately guarded. e) No. 10PM Tending Side - The rope sheave just before the coater was not adequately guarded. f) No. 11PM Tending Side - The nip point between the first press felt and the first roll in the first press was not adequately guarded. g) #5 PM, 2nd Press - Nip point at tending side of 3rd felt roll was not guarded. h) No. 5PM Backside - The oscillating shaker drive was not adequately guarded against contact. i) Groundwood North End Operating Level - The inrunning nip point on the cross feed belt conveyor and pulley was not adequately guarded. j) No. 5PM Basement Tending and Drive Side - The inrunning nip points on two felt stretch rolls were not guarded.
