29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained. When that ability is interrupted at any time, the operator did not safely stop operations requiring signals until it is reestablished and a proper signal is given and understood: On or about August 29, 2015, at the worksite located at the east side of Miramar Parkway & I-75, Miramar, FL. the employer did not ensure that proper signals were maintained at all times or the operations stopped during the change-out of wood cushion blocks thereby exposing employees to struck-by and caught-by hazards.
