29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. A.) Warren Railcar Service Inc.; throughout facility - On or about 8/19/15, and at times prior thereto, employees were potentially exposed to a trip and fall hazard from defects in the floor (walking/working surface). These defects included but were not limited to heaves, dips, pot holes, and buckles. The defected area extended from the entrance for the railcars to the end of the machining area.
