29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about October 21, 2011, and at times prior to that date, for four employees exposed to cave-in crushing hazards while working in a 24-foot deep excavation with vertical sidewalls at the ADM Benson-Quinn grain facility in Hebron, ND.
