29 CFR 1910.132(c): Personal protective equipment was not of safe design and construction for the work to be performed: a. During the week of November 2, 2015 and prior, the employer failed to ensure employees performing preventative maintenance on crane 304 were protected against a fall of approximately 40 feet through the main hoist drum rope opening by certifying/designating tie off locations. b. During the week of November 2, 2015 and prior, the employer failed to ensure employees performing preventative maintenance on crane 305 were protected against a fall of approximately 40 feet through the main hoist drum rope opening by certifying/designating tie off locations. c. During the week of November 2, 2015 and prior, the employer failed to ensure employees performing preventative maintenance on crane 307 were protected against a fall of approximately 40 feet through the main hoist drum rope opening by certifying/designating tie off locations. d. During the week of November 2, 2015 and prior, the employer failed to ensure employees performing preventative maintenance on crane 314 were protected against a fall of approximately 40 feet through the main hoist drum rope opening by certifying/designating tie off locations. e. On or about August 6, 2015 and prior, the employer failed to ensure employees performing preventative maintenance on crane 315 were protected against a fall of approximately 40 feet through the main hoist drum rope opening by certifying/designating tie off locations.
