29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: On or about June 24, 2014, at a job site located at 6476 Strip Ave. NW, Canton, OH, during carpentry and framing activities, the employer exceeded the load imposed on a mobile scaffold (scissors lift) guardrail by an employee standing on a guardrail to perform work activities
