29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about June 16, 2015, employees were required to perform maintenance on a compressor. The employer did not ensure that lockout devices were used to isolate all energy sources on the compressor, thereby exposing all employees to potential contact of energized of equipment.
