29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: A) Siferd Plumbing Heating & AC Services Co., LLC, conducting work at the private residence located at 6210 Township Road 254, Arcadia, OH 44804: On or about June 16, 2015, the employer failed to provide training to ensure that employees, who were required to enter a permit-required confined space, acquired the understanding, knowledge, and skills necessary for safe performance of the work. Hazards, associated with a permit-required confined space, were not identified before employees were required to enter the wastewater treatment systems tank in order to conduct repairs. Employees were exposed to the hazards such as, but not limited to, atmospheric hazards (hydrogen sulfide and carbon monoxide).
